Your Information

CYC Mentor or Staff Information

Who is your main point of contact or mentor at CYC? We may follow up with this individual and ask them to provide additional information about your unique qualities to add to your application materials for this award.

Student Photo

Ensure your face is visible and that the photo is appropriate to be shared for media and other marketing materials promoting your nomination or award.

Student Video or Essay

Please submit a short 2–4-minute video, or a 500–750-word essay, that answers the following prompts to help our judges know more about you:

  1. State your name and the school you are attending/attended as part of CYC.
  2. If you have already graduated, describe how you are currently pursing enrollment in higher education or training, enlistment in the military, or employment in the workforce.
  3. Describe how has CYC made a positive impact on your education, career, or life decisions.
  4. Describe why you believe you should receive this reward.
